Historic 58 Dads registered with the Idaho Putative Father Registry in 2017 – this is the highest number of Dads registering since the law went into action in 1985.

In order to claim rights of paternity, fathers of children born out-of-wedlock must commence proceedings to establish paternity AND file notice of his commencement of proceedings to establish his paternity with the Vital Records, Idaho Department of Health & Welfare. Acknowledgement of Paternity Affidavit   This is a SAMPLE Affidavit for the Biological Father to Complete. In this sworn statement the biological Dad consents to the recording of his name, date, and place of birth on the birth certificate. Dad’s signature must be signed in front of a Notary Public.
