For Fathers of Children Born Out-of-Wedlock
This is also known as the Idaho Putative Father Registry.
In order to claim rights of paternity, fathers of children born out-of-wedlock must commence proceedings to establish paternity AND file notice of his commencement of proceedings to establish his paternity with the Vital Records, Idaho Department of Health & Welfare. Acknowledgement of Paternity Affidavit This is a SAMPLE Affidavit for the Biological Father to Complete. In this sworn statement the biological Dad consents to the recording of his name, date, and place of birth on the birth certificate. Dad’s signature must be signed in front of a Notary Public.
